29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a.  On or about April 30, 2024, at the above addressed jobsite, an employee using a pneumatic nail gun was not wearing safety glasses, thereby exposing the employee to hazards associated with eye injuries.  b.  On or about April 30, 2024, at the above addressed jobsite, employees using a handheld powered skilsaw were not wearing safety glasses, thereby exposing the employees to hazards associated with eye injuries.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  a.  On or about April 30, 2024, at the above addressed jobsite, an employee performing residential carpentry operations, twenty-one (21) feet four (4) inches above ground, was not protected from falls, thereby exposing the employee to fall hazards.  b.  On or about April 30, 2024, at the above addressed jobsite, an employee performing residential carpentry operations, twenty-one (21) feet two (2) inches above ground, was not protected from falls, thereby exposing the employee to fall hazards.  c.  On or about April 30, 2024, at the above addressed jobsite, an employee performing residential carpentry operations, eight (8) feet two (2) inches above ground, was not protected from falls, thereby exposing the employee to fall hazards.

On or about April 30, 2024, at the above addressed jobsite, the ladder used to access and egress the upper working level of the structure was a six-foot A-Frame ladder that was closed and leaned against structure, thereby exposing employees to fall hazards.
