29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about May 29, 2024, at the previously addressed job site, one employee was observed collecting debris on the ground from roofing materials being torn off and being thrown from the roof by other employees, the employee on the ground was walking amongst the debris as it was being thrown off the roof while not wearing a protective helmet to prevent their head from being struck by falling debris.

29 CFR  1926.252(a): An enclosed chute of wood, or equivalent material, was not used where materials were dropped more than 20 feet to points lying outside the exterior walls of the building(s):  On or about May 29, 2024, at the previously addressed job site, employees were observed removing shingles and other materials from the roof of a apartment complex.  Employee were dropping the shingles and other materials at times from heights greater than twenty feet from the exterior roof to the ground outside without the use of enclosed chute.

29 CFR  1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:(a)   On or about May 29, 2024, at the previously addressed job site, employees were observed using an four different extension ladder to access the top of the roof of an apartment building for roofing activities, which did not have side rails that extend 3 feet past the landing surface (roof).

29 CFR  1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.(a)  On or about May 29, 2024, at the previously addressed job site, employees were observed using an extension ladder to access the top of the roof of an apartment building for roofing activities, the ladder had a damaged rung that was cracked and bent at a height of approximately 4 feet and 5 tenths of a foot the rung was the fifth rung from the ground.

The JJ Renovations Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number1628034, citation number 1, item number 1 and was affirmed as a final order on 01-27-2023, with respect to a workplace located at 705 Jackson Blvd Oak Park IL 60304.

29 CFR  1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about May 29, 2024, at the previously addressed job site, five employees were observed performing roofing activities on a  a residential structure at a height of greater than 6 feet without the use of personal fall protection or guard rails to keep them from falling from heights of greater than six feet.   The JJ Renovations Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number1628034, citation number 1 , item number 3 b and was affirmed as a final order on 1628034, with respect to a workplace located at 705 Jackson Blvd Oak Park IL 60304.
