29 CFR  1910.147(f)(2)(i): Whenever outside servicing personnel are to be engaged in activities covered by the scope and application of this standard, the on-site employer and the outside employer shall inform each other of their respective lockout or tagout procedures.   On or about May 20, 2024, to May 24, 2024, at an establishment located at 444 Highland Drive in Sheboygan, Wisconsin, in the Powerhouse; the on-site employer, Kohler Co., and MCT Services LLC did not inform each other of their respective group lockout procedures. As a result, three MCT Services LLC employees were exposed to potential hazardous energy while working on the South Cooling Tower System - East Cell. All of the employees did not apply a lock to the group lockbox located in the Powerhouse Control Room or verify that the five lockout points were locked out.

Each authorized employee shall affix a personal lockout or tagout device to the group lockout device, group lockbox, or comparable mechanism when he or she begins work, and shall remove those devices when he or she stops working on the machine or equipment being serviced or maintained.  On or about May 20, 2024, to May 24, 2024, at an establishment located at 444 Highland Drive in Sheboygan, Wisconsin, in the Powerhouse; employees were exposed to potential hazardous energy when each of the four employees working on the South Cooling Tower System - East Cell did not apply a lock to the group lockbox located in the Powerhouse Control Room. Three employees were working under one lock that the superintendent applied to the lockbox.
