At a worksite located at 23731 Missouri Highway 11, Kirksville, Missouri 663501, employees were exposed to caught between and crushed by hazards while operating power industrial trucks. The employer did not ensure that employees who were operating powered industrial trucks in the workplace had successfully completed training consisting of a combination of formal instruction, practical training, and evaluation of the operator's performance in the workplace.

29 CFR 1910.1200(h)(1):Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: At a workplace located at 23731 Missouri Highway 11, Kirksville, MO 63501 employees were exposed to chemical products such as, but not limited to, ZepInc FOAMING VEHICLE WASH, SPLASH Original Blue Windshield Wash, and CHS Propane. Health effects from these products can cause eye, skin, and respiratory irritation. The employer did not provide effective information and training on hazardous chemicals in their work areas.
