29 CFR  1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided.  On May 24, 2024, the employer did not provide an extension ladder where employees could access and egress the residential building roof level, exposing them to potential fall hazards.

29 CFR  1926.1052(c)(1)(i): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail.   On May 24, 2024, inside the residential home, the employer and employees utilize a stairway under construction to access and egress the levels exposing employees to potential fall hazard approximately 10 feet from a 13-step stairway.

29 CFR  1926.102(a)(1): Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:   On May 24, 2024, employees installing decking material use pneumatic nail guns without wearing eye protection were exposed to struck-by hazards and eye injuries.  Martinez Vazquez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1711409, citation number 01, item number 002 and was affirmed as a final order on February 22, 2024, with respect to a workplace located at 1209 Roundwash Way, Midlothian, Texas 76065.

On May 24, 2024, employees engaged in decking activities without fall protection were exposed to unprotected fall hazards of approximately 12 feet to the ground below.
