29 CFR 1926.451(g)(4)(iv): Midrails were not installed at a height approximately midway between the top edge of the guardrail system and the platform surface: a) Jobsite, 419 Melwood Avenue, Pittsburgh, PA: On May 30, 2024, the cross point of the X brace that was being used as the midrail on the 14th level of scaffolding did not meet the height requirements of midway between the toprail and the working surface.

29 CFR 1926.454(c)(3): Where inadequacies in an affected employee's work involving scaffolds indicate that the employee has not retained the requisite proficiency: a) Jobsite, 419 Melwood Avenue, Pittsburgh, PA: On May 30, 2024, during the scaffold inspection process and green tag initializing, the Lead & Working Foremen and the Labor Foreman/Scaffold Erector failed to recognize the guardrails on the 14th level did not meet the height requirements for a standard guardrail system and the toeboards were missing.
