29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a. At Building 3 - Apartment 3309, balcony located at 440 Burbank Avenue, Nocatee, FL 32081: On or about May 30, 2024, the employer exposed three employees to fall hazards, in that fall protection was not utilized by employees while unloading waterproofing materials from the lull.  The employees were being exposed to a 21 feet 8-inch fall hazard.

Agantic Group, LLC, located at 10752 Deerwood Park Blvd, - Suite 330, Jacksonville, FL 32246: On or about May 30, 2024, the employer failed to report calendar year 2023, to electronically submit information from the OSHA 300 Form 300A or equivalent by March 2, 2024.  The establishment employer 92 employees and was classified in the North American Industrial Classification system as 238110 during calendar year 2023.

At a temporary job site located at 440 Burbank Avenue, Nocatee, FL 32081: On or about May 30, 2024, the employer exposed employees to crushing hazards, in that the employer did not train and certify that each employee operating a lull was trained to do so.
