29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about 05/30/2024; throughout the establishment: Lockout/Tagout (LOTO) procedures were not developed and documented when employees performed servicing and maintenance activities such as, but not limited to, changing pinch valve on Sand Blaster, change ball screw on #27 Cincom Citizens Lathe, changing belt on Rockwell/Delta belt sander, changing hydraulic hose on #19 DMG Mori milling machine, exposing employees to amputation hazards. ABATEMENT DOCUMENTATION REQUIRED

29 CFR 1910.147(c)(4)(ii): Energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): a) On or about 05/30/2024; throughout the establishment; Employees were exposed to the unexpected energization or start-up of equipment such as, but not limited to: SS White Airbrasive Jet Machine Unit Model-HME Sand Blaster, #27 Cincom Citizens Lathe, Rockwell/Delta belt sander, #19 DMG Mori milling machine when performing servicing and maintenance activates and the energy control procedures did not include specific procedural steps for: 1) A specific statement of the intended use of the procedure, 2) Shutting down, isolating, blocking, and securing machines or equipment to control hazardous energy, 3) The placement, removal, and transfer of lockout devices or tagout devices, a description of who has responsibility for them, 4) Testing a machine or piece of equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures. ABATEMENT DOCUMENTATION REQUIRED
