29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Examples of guarding methods are - barrier guards, two-hand tripping devices, electronic safety devices, etc.  On or about May 08, 2024, the employer did not ensure that the pinch point on a ICN 2500 Semi-Automatic Pad Printer SN: PO711MO3, Machine number 10, was guarded to prevent the operator from having any part of her body in the danger zone during the operation cycle.

29 CFR  1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  On or about May 8, 2024, an employee sustained a partial left index finger amputation while working on a ICN 2500 Semi-Automatic Pad Printer machine number 10. The employer failed to report the amputation within 24 hours. OSHA was not notified until May 29, 2024, by employee legal representation.

29 CFR  1910.132(d)(2): The employer did not verify that the required workplace hazard assessment(s) had been performed through a written certification which included the requirements in paragraph (d)(2) of this section:  On or about May 31, 2024, the workplace hazard assessment had not been put in writing and certified with the person conducting the evaluation and the dates the hazard assessment was completed.  Safety glasses, gloves,  and ear plugs are required when performing certain job tasks.
