29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace. At a worksite located at 641 SE Tenth Rd, Lamar, Missouri 64759, employees were exposed to caught between and crushed by hazards while operating power industrial trucks. The employer did not ensure that employees who were operating powered industrial trucks in the workplace had successfully completed training consisting of a combination of formal instruction, practical training, and evaluation of the operator's performance in the workplace.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about May 24, 2024: Employees engaged in pallet manufacturing activities at 641 SE Tenth Rd, Lamar, Missouri 64579, were exposed to pinch point hazards in that the employer did not ensure employees were protected from unguarded point of operation while operating a single head notching machine to produce parts and pieces for wood pallet manufacturing.
