29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems: (a) On or about May 28, 2024, the the employer did not ensure that employees utilized a fall protection system when cutting a 3-foot square opening on top of the low slope roof of the structure for a heating cooling and ventilation system installation. This condition exposed employees to a fall of approximately 20 ft. the the lower level.

29 CFR 1926.761(b): The employer did not provided a fall hazard training program for employees engaged in steel erection activities: (a) On or about May 28, 2024, the employer did not ensure that each employee engaged steel erection operations were trained on fall hazard identification, recognition, and methods to guard against falling. Employees did not utilize fall protection when they cut a 3-foot square opening in a low slope steel deck for the installation of a heating cooling and ventilation system. This condition exposed employee to a fall of approximately 20 feet to the lower level.
