29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) On or about June 4, 2024, at the facility located at W219 S7768 Crowbar Dr., Muskego, WI, the employer did not ensure employees utilized procedures of controlling hazardous energy sources while performing servicing and maintenance activities on equipment including, but not limited to, CNH Industrial America C227 Loader, which had multiple energy sources. The employer had not developed, documented and utilized machine specific procedures, this exposed employees to caught between and struck-by hazards.
