At 1215 Imeson Park Blvd in Jacksonville Florida. On or about June 4, 2024, in the 12-foot deep trench, the employer exposed employees to emergency egress hazards in that employees were not provided use of a stairway, ladder, ramp, or other safe means of egress in event of accident, soil collapse, or medical emergency.

At 1215 Imeson Park Blvd in Jacksonville Florida. On or about June 4, 2024, in the 12-foot deep trench, the employer exposed employees to crushing, struck-by, and engulfment hazards from spoil piles that were placed on the top of the leading edge of the trench sidewalls without any setback distance.
