29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  a) Jobsite located at or near 84246 Pine Street, Philadelphia, PA- On or about June 4, 2024, the employer did not ensure that each working level of a scaffold was fully planked or decked between the front uprights and guardrail supports. Employees conducting masonry activities were utilizing a scaffold platform that was not fully planked, thereby exposing employees to fall hazards or approximately 13 to 26 feet.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee shall be protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  a) Jobsite located at or near 84246 Pine Street, Philadelphia, PA- On or about June 4, 2024, the employer did not ensure that employees conducting masonry activities from a tubular frame scaffold were provided fall protection systems or guardrail systems, thereby exposing employees to fall hazards of approximately 13 to 26 feet.
