29 CFR  1910.147(a)(2)(ii)(B): The employer did not ensure proper lock out tag out devices were utilized in order to prevent  an employee from placing any part of his or her body into an area on a machine or piece of equipment where work is actually performed upon the material being processed (point of operation) or where an associated danger zone exists during a machine operating cycle.   a) On or about June 3, 2024, at the job site: an employee was exposed to an amputation hazard while demonstrating the use and functions of a Masipack, Ultra VS 4.0 300 Bagger Machine (serial number 20240031) to a customer by bypassing the door interlock and placing his hand on top of the seal/cut jaws, in that, locked out tag out devices were not used.
