29 CFR  1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:   At the job site located at 15634 E Rockhill Ct, Wichita, KS 67230, employee(s) utilizing a Sky Track telehandler, model number 8042 to support wooden platform and conduct roofing activities from it were exposed to fall hazards. The platform was not designed or approved by the manufacturer of the Sky Track. An employee was exposed to fall hazards of approximately 13.5 feet to the ground below.

29 CFR  1926.451(g)(1)(vii):Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  At the job site located at 15634 E Rockhill Ct, Wichita, KS 67230, employee engaged in residential roofing activities on a Sky Track telehandler with model number 8042, was exposed to fall hazards in that no personal fall arrest systems or guardrail systems was utilized. Employee was working at a height of approximately 13.5 feet without the use of any fall protection.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  At the job site located at 15634 E Rockhill Ct, Wichita, KS 67230 employees engaged in residential roofing activities were exposed to falls from approximately 13.5  feet, in that an aluminum ladder used to access the working level of the structure did not extend at least 3 feet above the landing surface.
