29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): (a) Rough Mill department, door cell area; On or about June 3, 2024, the service and maintenance task of adding tape around the top cover of the fence assembly to prevent dust/debris from escaping, was performed on the Powermatic Model 27 wood shaper, without the equipment being unplugged or isolated from the energy source.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: (a) Mill department; On or about May 3, 2024, and dates prior, the service and maintenance task of replacing the cutters on the SCMI Model T130N wood shaper, was performed without affixing a lockout or tagout device to the equipment's energy source.
