29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s):  (a) Rough Mill department, door cell area; On or about June 3, 2024, the service and maintenance task of adding tape around the top cover of the fence assembly to prevent dust/debris from escaping, was performed on the Powermatic Model 27 wood shaper, without the equipment being unplugged or isolated from the energy source.
