29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: a). On or about June 05, 2024, at 12-16 30th Ave, Astoria, NY 11102. Employees working on a water and sewer installation inside a trench approximately 7-feet in depth did not have a safe mean of egress located inside the trench. This hazard exposed employees to cave-in hazard. Note: Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system. a). On or about June 05, 2024, at 12-16 30th Ave, Astoria, NY 11102. Employees working on the installation of a water and sewer line inside a trench measured 7-ft in dept, 6ft long and 4ft wide that was missing shoring were exposed to cave-ins hazard. Note: Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.
