29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about June 5, 2024, employees engaged in roofing activities, located at 301 E 5th St., Fremont, NE 68025, were exposed fall hazards in that the unprotected employees were conducting roofing work approximately 12-feet 8-inches above the ground below without fall protection.

29 CFR 1926.1053(b)(1):Where (a) portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device was not provided to assist employees in mounting and dismounting the ladder. On or about June 5, 2024, employees engaged in roofing activities, located at 301 E 5th St., Fremont, NE 68025, were exposed fall hazards in that they were accessing and egressing a roof approximately 12-feet 8-inches ground to eave from an extension ladder that did not extend 3-feet above the surface that was being accessed.
