29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about June 5, 2024, employees engaged in roofing activities, located at 747 N Irving St, Fremont, NE 68025, were exposed fall hazards in that they were accessing and egressing a roof where the ground to eave measured approximately 9-feet one inch using an extension ladder that was not extended 3-feet above the surface being accessed.
