29 CFR 1926.1053(b)(1): The ladder extension was positioned such that deflection under a load, by itself, would cause the ladder to slip off its support. On or about June 6, 2024, at multiple places around the worksite, workers were using ladders to access the roof that were not extended three feet: Instance A. At the northwest corner of the structure, the Werner ladder was not extended 3' over the access point of the roof. Instance B. At the southeastern corner of the structure, the Werner ladder was not extended 3' over the access point of the roof. Instance C. At the east side of the structure, the Werner ladder was not extended 3' over the access point of the roof.

On or about June 6, 2024, at multiple places on the two-story roof, and working higher than 6' above the ground, workers were conducting roofing work with no fall protection. Yovani Olea was previously cited for a violation of the Occupational Safety and Health Standard29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1643356, citation number 1, item number 1 and was affirmed as a final order on 04/06/2023 with respect to a workplace located at 1054 Navasota St, Webster, TX 77589.
