29 CFR 1926.451(e)(1): Cross braces were used as a means of access:   a) On or about 06/05/24 at jobsite 38-29 30th Street. Long Island City, NY south side of the property.     Employees working from a fabricated frame scaffold were exposed to the hazard of falling approximately 18-ft. to the ground. The employees were climbing the system cross braces as means of accessing scaffold work platforms.

Long Island City, NY south side of the property.     Employees erected a fabricated frame scaffold without the supervision and direction of a competent person. The scaffold was erected without an access ladder or guard rail systems.

Long Island City, NY south side of the property.   Employees installing siding from the fabricated frame scaffold were exposed fall hazards of approximately 18-ft to the ground. Guardrails or personal fall arrest systems were not used.

29 CFR 1926.502(d)(6)(i): A snaphook not of the design type was engaged directly to webbing, rope or wire rope:  a)           On or about 06/05/24, at the site of 38-29 30th Street. Long Island City, NY south side of the property.  Employees were installing siding from the fabricated frame scaffold.  Employees were using a personal fall arrest system consisting of a full body harness and a 6' shock absorbing synthetic webbing lanyard.  The lanyard was connected directly to the frame of the scaffold by wrapping the lanyard around the scaffold frame and connecting the snaphook back to the lanyard.
