29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a)On or about June 6, 2024, and at times prior thereto, at the Catron Drive jobsite, where employees without hard hats were working inside a trench below other workers and were exposed to head injuries from rocks and hand tools in close proximity to the edge of the trench.

29 CFR  1926.651(h)(1): Employees were working in excavations in which there was accumulated water, or excavations in which water was accumulating, and adequate precautions had not been taken to protect employees against the hazards posed by water accumulation:  (a) On or about June 6, 2024, and at times prior thereto, at the Catron Drive jobsite, where employees were installing a water tap and were exposed to a cave in hazard because water had been allowed to accumulate at the bottom of the trench and had not been pumped out.

29 CFR  1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c):  (a) On or about June 6, 2024, and at times prior thereto, at the Catron Drive jobsite, where employees were exposed to cave in/ engulfment hazards while working inside an unprotected trench, adjacent to a roadway, and with water accumulated at the bottom.

29 CFR  1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired:  (a) On or about June 6, 2024, and at times prior thereto, at the Catron Drive jobsite, where employees inside a trench were using the broken remains of an extension ladder to access the trench and were exposed to fall hazards.
