29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access: location: scaffold access leading to sidewalk shed on or about: 6/5/24 a) Employees were working from a tubular welded frame scaffold. The employees were not provided with adequate access. Employees were using an extension ladder to access the scaffold and had to step down approximately 30 inches to get from the ladder to the scaffold platform. location: scaffold access from 4th floor on or about: 6/5/24 b) Employees were working from a tubular welded frame scaffold. The employees were not provided with adequate access. Employees were using the 4th floor window to access the scaffold and had to step up 36 inches to get from the 4th floor to the scaffold platform. NOTE: Because abatement of this violation is already documented in the inspection casefile, the employer need not submit certification or documentation of abatement for this violation as normally required by 29 CFR 1903.19.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level: location: front of building on or about: 6/5/24 a) Employees, engaged in brickwork from a scaffold platform up to 50 feet above the ground, were not provided with adequate fall protection. The top endrails were 36 inches above the platforms.
