29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a.  Near Northwest Wall by "Midwest Automation" CS 5230:  On or about June 6, 2024, and at times prior, the employer exposed employees to laceration hazards, in that the employer did not maintain effective guarding on a "Maxx Air" industrial drum fan where there was a 2-3/8 inch opening on the face of the fan guard 4 inches from the fan blade where employees operate saws for the production of countertops.  b.  Cap Area:  On or about June 6, 2024, and at times prior, the employer exposed employees to laceration hazards, in that the employer did not maintain effective guarding on a "Patton" industrial pedestal fan where there was a 4-1/4 inch opening on the face of the fan guard where employees perform tasks for manufacturing countertops.

Northwest of Production Area near "Midwest Automation" CS 5230:  On or about June 6, 2024, and at times prior, the employer exposed employees to electric shock hazards, in that the employer did not ensure that an Underwriters Laboratories listed relocatable power tap was connected directly to a branch circuit receptacle.  b.  Northwest of Production Area near "Midwest Automation" CS 5230:  On or about June 6, 2024, and at times prior, the employer exposed employees to electric shock hazards, in that the employer did not ensure that an Underwriters Laboratories listed relocatable power tap was not daisy chained with flexible extension cords.  c. 29 CFR  1910.305(g)(1)(iv)(A):  Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  a. Production Area, near two "Midwest Automation" CS 5230:  On or about June 6, 2024, and at times prior, the employer exposed employees to electrical shock hazards, in that the employer used an extension cord as a substitute for fixed wiring to provide power to cord and plug connected overhead lighting while employees operate saws for the production of countertops.  b.  Production Area northwest wall at "Midwest Automation" CS 5230: On or about June 6, 2024, and at times prior, the employer exposed employees to electrical shock hazards, in that the employer used an extension cord as a substitute for fixed wiring to provide power to cord and plug connected overhead lighting.  c.  Production Area at miter saws: On or about June 6, 2024, and at times prior, the employer exposed employees to electrical shock hazards, in that the employer used extension cords as a substitute for fixed wiring to provide power to cord and plug connected overhead lighting, a "Ryobi" model TS1346 chop saw and a "Maxx Air" industrial drum fan while employees operate saws for the production of countertops.

Production Area; adjacent to "Ryobi" miter saw:  On or about June 6, 2024, and at times prior, the employer exposed employees to electrical shock hazards, in that strain relief was not provided on an extension cord used to provide electrical current from a relocatable power tap to a "Maxx Air" industrial drum fan while employees perform tasks associated with the production of wood and laminate countertops.  b.   Production Area; CAP Area:  On or about June 6, 2024, and at times prior, the employer exposed employees to electrical shock hazards, in that strain relief was not provided on the cord of a "Dayton" pedestal fan connected to a double gang receptacle box used as pendant receptacle while employees perform tasks associated with the production of wood and laminate countertops.

29 CFR  1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical:  a.  Cap Area:  On or about June 6, 2024, and at times prior, the employer exposed employees to hazards associated with flammable chemicals, in that the employer did not ensure that pump cans containing "Wilsonart" 110 Adhesive Solvent were labeled for chemical content where employees use the chemical to clean surfaces in the production of wood and laminate countertops.
