29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a)Single Ply Roofing, LLC, at 2908 South Louise Avenue, Sioux Falls, SD 57106:  On or about and at times prior to June 6, 2024, the employer did not ensure an adequate fall protection system was in place while employees were conducting roofing operations on a flat roof multi-family residential property.  This condition exposed employees to fall hazards greater than six feet.

This condition exposed employees to fall hazards.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a)Single Ply Roofing, LLC, at 2908 South Louise Avenue, Sioux Falls, SD 57106:  On or about and at times prior to June 6, 2024, the exposing employer did not ensure that the Werner 40 foot extension ladder being utilized to access the roof of the multi-family residential property was extended at least three feet above the upper landing surface.  This condition exposed employees to fall hazards.

This condition exposed employees to various hazards such as fall hazards, ladder hazards, and a lack of training documentation.
