29 CFR  1910.36(g)(2):Exit access was not at least 28 inches wide at all points:  a) MRF Building - C Line - Employees were exposed to emergency egress hazards in that the employer did not maintain 28 inches wide exit access at all points. Aisleways which lead to emergency exit routes were blocked with a permanently place debris chute and only offered approximately 7 inch wide exit access. This condition was observed on or about June 5, 2024.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR  1910.37(a)(3): Exit routes were not free and/or unobstructed, equipment was placed permanently within the exit route:  a) MRF Building - Transfer Station - C Line - Employees were exposed to emergency egress hazards in that the employer did not maintain that exit routes are unobstructed. Aisleways which lead to emergency exit routes were blocked with a permanently place debris chute. This condition was observed on or about June 5, 2024. 29 CFR  1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders was not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in Sec. 1910.6. Section 3.3.1 of the CGA Pamphlet P-1-1965 refers to storing cylinders in accordance with CGA and NFPA standards. The CGA Pamphlet P-1-1991 Section 3.7.4.1 states that "All compressed gas cylinders in service or in storage shall be secured to prevent from falling.":  a) Welding Shop - Employees are exposed to fire/explosion hazards in that the employer allows worker to use Oxygen and Acetylene torch set-up in a free standing position while in service and not secured to an approved cart or structure. This condition was observed on or about June 5, 2024.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. 29 CFR  1910.303(b)(7)(i): Unused openings in boxes, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment:  a) Welding Shop - The employer did not ensure that unused openings in the energized circuit breaker panel (120/240V) was effectively closed, the energized conductor (buss bar) was not guarded against accidental contact.
