On or about June 12, 2024, the employer provided a general company safety policy for warehouse operations outlining emergency action plans for employees effective 6/12/24. No written emergency action plan was in place for employees to review prior to the inspection with the employer on 6/12/24.

On or about June 12, 2024, the employer did not ensure portable fire extinguishers located were readily accessible to employees. A portable fire extinguisher was found with a table placed in front, measuring approx. three feet in width from the portable fire extinguisher to the employee accessibility point.
