29 CFR 1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about May 31, 2024, the employer did not ensure the Point Source 8 Dust Collector Machine Cleaners performing service and maintenance activities by unjamming materials stuck in the discharge spout and rotary valve applied and followed the established energy control procedures by; turning off or shutting down the Point Source 8 Dust collector (per 1910.147(d)(2), isolating the Point Source 8 Dust collector from the electrical and pneumatic energy sources {(d)(3)}, affixing lockout devices(s) to the energy isolation device (s) {(d)(4)(i)} and verifying de-energization and isolation had been accomplished {(d)(6)} prior to unjamming the Discharge spout guard and rotary valve.
