29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects: a) On or about June 10, 2024, the employer did not ensure that employees untying bags of graphite (35"x35"x64" 3000-lbs capacity) overhead to load the fine bagger machine hopper using a bridge crane (Serial number 36049, Capacity 7.5 tons) on the powder line were protected from falling objects by use of a protective helmet.

29 CFR 1910.303(b)(1)(ii): Electrical equipment was not free from recognized hazards, based on the mechanical strength and durability, including, for parts designed to enclose and protect other equipment, the adequacy of the protection thus provided: a) On or about June 10, 2024, employees moving bags of graphite (35"x35"x64" 3000-lbs capacity) to the Old AC Mill with powered industrial trucks were exposed to the live conductors of up to 480-volt from the Old AC Mill control panel; the employer did not ensure that the Old AC Mill 480-volt load break switch kept the control panel door secured to enclose and protect the electrical conductors while energized.

When normally enclosed live parts are exposed for inspection or servicing, the working space, if in a passageway or general open space, shall be suitably guarded. a) On or about June 10, 2024, the employer did not ensure the working space in front of the following electrical panels throughout the west manufacturing building was kept clear of obstruction: 1. Blue A/C Electrical disconnect 480-volts 2. Main Emergency service 480-volts. 3. Pilot Plant disconnect 120/208-volts. 4. Power Panel J4 120/208-volts. 5. Power Panel K6 120/208-volts.
