29 CFR  1910.176(a): Use of mechanical equipment. Where mechanical handling equipment is used, sufficient safe clearances shall be allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways shall be kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways shall be appropriately marked.   On or about June 10, 2024, in the basement near the Surefire Plus Hydraulic Unit area, the aisle where the tow motor drove through was not kept in good repair. The grating and metal plates that sat over the small trench would become displaced after tow motors drove over them. This caused an opening in the floor, or the grates and metal plates would protrude out of the trench.
