29 CFR  1926.451(e)(1):When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access.  a) On or about June 11, 2024, at the site, the employer allowed employees to use the cross braces as an access ladder to work on a scaffold, exposing employees to fall hazards.

29 CFR  1926.451(f)(14): Makeshift devices, such as but not limited to boxes and barrels, were used on top of scaffold platforms to increase the working level height of employees:   a) On or about June 11, 2024, at the site, employees were exposed to fall hazards, in that they were using a bucket to raise the height on a scaffold that was not fully planked to work on a soffit approximately 12 feet high.  b) On or about June 11, 2024, at the site, employees were exposed to fall hazards, in that they were using a cinder concrete block to raise the height on a scaffold that was not fully planked to work on a soffit approximately 12 feet high.
