OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire and explosion hazards in the event of catastrophic equipment failure while performing pneumatic pressure tests on a bare screw compressor within a Screw Compressor Test Chamber: a) Compressor Assembly Building, Compressor Test Area Compressor Assembly Building, Compressor Test Area ? On or about June 11, 2024, employees performed a pneumatic pressure test of a bare screw compressor within a screw compressor test chamber when a combustion event within the test chamber unexpectedly and unforeseeably occurred. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.212(a)(2): Machine guard(s) on machines were so installed that they offered accident hazards in themselves: a) Compressor Assembly Building, Compressor Test Area - The design and installation of the yellow sheet metal barrier located in front of the Screw Compressor Test Chamber's pressure relief ports exposed employees to struck-by hazards in the event of a catastrophic overpressurization event within the test chamber. Employees suffered serious injuries not limited to severe lacerations and bruising and fractured bones after being struck by the barrier after it blew off the test chamber as projectile towards them, on or about June 11, 2024. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
