29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) Large Toolbox Shear: On or about June 7, 2024, employees were exposed to cut-by hazards while operating the large toolbox shear, due to working from the rear of the shear while the shear blades were exposed. b) Old Tube Bender, Frame Department; On or about June 7, 2024, an employee was exposed to struck-by hazards while operating the old tube bender due to the close proximity of the controls to the point of operating. Abatement certification and documentation are required.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Old Tube bender, Frame Department: On or about June 11, 2024, employees were exposed to struck-by hazards when walking in the area of the bender due to a lack of guarding to prevent the end of the tubing from striking employees when the machine is activated. Abatement certification is required.
