OSH Act of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing, or likely to cause, death or serious physical harm in that employees were exposed to crushed by hazards, due to the unsafe operation of automotive lifts: Service Area - Auto lifts were operated unsafely and/or with defects, in the following instances: a) Bay #2, Post Rotary Lift (SN: JB920J0011) - Lift had not been inspected annually. Cables needed to be adjusted and/or replaced. Safety stickers were covered by electrical equipment. b) Bay #3, Two Post Challenger Lift (SN: 274366) - Lift had not been inspected annually. Swing arm restraints on 3 of 4 swing arms were nonoperational. Swing arm extension stops were nonoperational. Hydraulic oil leak on driver side cylinder. c) Bay #4, Forward Scissor Lift (SN: KYV17F0003) - Lift had not been inspected annually. Hydraulic oil leaking out of overflow ports on both cylinders.
