29 CFR 1910.22(c): The employer did not provide, and ensure each employee uses, a safe means of access and egress to and from walking-working surfaces. On or about May 30, 2024, at 8208 South Kohler Road, Apple Creek, OH 44606, the employer failed to ensure employees climbing on top of the Woodpecker Nailing Machine located on the South Wall in the Assembly Department had a safe means of access and egress. Employees utilize a side frame rail to gain access to the 31 inch table top.

29 CFR 1910.28(b)(3)(ii): The employer did not ensure that each employee be protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems: On or about May 30, 2024, at 8208 South Kohler Road, Apple Creek, OH 44606, the employer failed to ensure employees climbing on top of the Woodpecker Nailing Machine located on the South Wall in the Assembly Department was protected against employees falling into a hole in the machine which had openings of 18 and 9 inches in width.
