29 CFR 1926.453(b)(2)(v):A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: (a) 1301 West Whitehall Road, State College, PA - Barn - Employees worked from a Genie TZ-50 trailer mounted boom aerial lift without a harness and lanyard attached to the basket exposing the employee to a fall of approximately 22 feet to grade, on or about June 12, 2024. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.102(a)(1):The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. (a) 1301 West Whitehall Road, State College, PA - Barn - Employees did not don safety glasses while using chainsaws to cut through barn beams and were exposed to flying debris and ricochet nails, on or about June 12, 2024. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
