On or about June 12, 2024, at the above addressed jobsite, employees performing residential roof repair activities from a Genie TZ 50 trailer mounted boom lift, greater-than twenty-five (25) feet zero (0) inches above ground, were not utilizing personal fall protection equipment, thereby exposing the employees to fall hazards.
