29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: Employee(s) were exposed to fall hazards. This was observed on or about June 12, 2024, at 21113 Hartman, Elkhorn, Nebraska 68022. Employees were exposed to fall hazards in that seven employees were using a portable ladder to access the roof which did not extend at least 3 feet above the roof surface.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of �1926.502(k) developed and implemented. Employee(s) were exposed to fall hazards. This was observed on or about June 12, 2024, at 21113 Hartman, Elkhorn, Nebraska 68022. Employees were exposed to fall hazards in that seven employees were working on a roof approximately 12 feet from the ground without fall protection. Custom Dream Contracting LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1426274, citation number 1, item number 2 and was affirmed as a final order on November 4, 2019 with respect to a workplace located at 2005 91st St, Omaha, Nebraska 68124.
