1926.503(a)(2)(ii) The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used: On or about June 10, 2024, at the previously addressed job site, employees were not trained in the correct procedures for installing covers.

29 CFR 1926.502(i)(4):All covers were not color coded or marked with the word "HOLE" or "COVER" to provide warning of the hazard: An employee from Duggan Painting went up to the second floor to clean the area in preparation for painting. At the time there was only a piece of plywood that was covering the hole. The plywood was not marked in any way to denote that it was covering a hole. The employee lifted the piece of plywood believing it was debris and fell through the hole 17 feet to the ground receiving multiple injuries.
