29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. a) On or about June 12, 2024, the employer did not conduct frequent and regular inspections of the jobsite. As a result, employees were exposed to overhead struck-by hazards without proper head protection (hardhat) in that employees were cleaning up the ground area surrounding the jobsite while overhead roofing work was being conducted.

29 CFR 1926.100(a):Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets. a) On or about June 12, 2024, the employer did not ensure that employees were wearing hard hats while cleaning up roof debris from the jobsite while overhead work was being performed. As a result, employees were exposed to overhead struck-by hazards.
