29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: a) Norfolk Naval Base Building V-61-9349 4th Ave. Norfolk VA 23511 on or about 06/06/ 2024, procedural steps were not clearly and specially outlined to control the multiple potential energy sources needed to deenergized and restrained components of the HVAC unit prior to the conducting the monthly planned maintenance as a result an employee's left ring finger was amputated. Notice 1, Item 1, 29 CFR 1910.147(c)(4)(ii)(B) as required by 29 CFR 1960. (8): The employer failed to implement an energy control procedure that specifically outline steps to control hazardous energy for equipment. The employer was cited for a violation of the same standard and/or same hazard on February 02, 2022, Inspection #1577651, April 08, 2022, of final order/settlement and means of abatement. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.147(d)(5)(i): All potentially hazardous stored or residual energy was not relieved, disconnected, restrained or otherwise rendered safe after the application of lockout or tagout devices to energy isolating devices: a) Norfolk Naval Base Building V-61-9349 4th Ave. Norfolk VA 23511 on or about 06/06/ 2024, the fan that connect to the motor pulley by belts in both the supply and return air handlers were not restrained to prevent potential movement caused by kinetic energy, thus causing an amputation. Notice 1, Item 2, 29 CFR 1910.147(d)(5)(i) as required by 29 CFR 1960. (8): The employer failed to implement an energy control procedure that specify all potentially hazardous stored or residual energy to relieve, disconnect, restrain or otherwise render safe after the application of a lockout or tagout devices to energy isolating devices: The employer was cited for a violation of the same standard and/or same hazard on February 02, 2022, Inspection #1577651, April 08, 2022, of final order/settlement and means of abatement.
