1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches were not equipped with a starrail system along each unprotected side or edge. (a) New Carbon Activation Furnace Room: On June 13, 2024, employees used a stairway that was 35.5 inches wide with 17 risers to access an elevated platform 13 feet 5 inches above ground level. The stairway was not equipped with stair rail system on the open side exposing employees to a fall hazard.

29 CFR 1910.28(b)(3)(i): The employer did not ensure that each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following methods listed in 29 CFR 1910.28(b)(3)(i)(A)-(D): (a) Production Line: On June 13, 2024, employees walked past an unprotected hole on their way to the time clock that was measured 25 feet above the lower level exposing employees to a fall hazard.
