29 CFR 1910.212(a)(1): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:        a) Tool Room - On June 5, 2024, the employer did not ensure that the rotating chuck on the Victor lathe, was equipped with a chuck guard to prevent injury.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

The receptacle boxes were installed to provide electricity to equipment such as but not limited to vacuums.        In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.305(j)(1)(ii): Portable hand-lamps supplied through flexible cords were not equipped with a handle of molded composition or other material approved for the purpose, and a substantial guard attached to the lamp-holder or the handle:      a) Self Assembly Area - On June 5, 2024, the employer provided three portable hand lamps with a metal shell, non-insulated handles, and without a guard for employees to use when illumination was needed on the light assembly area.      No abatement certification or documentation is required for this item.
