29 CFR  1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  (a)  On or about June 13, 2024, the employer did not conduct an inspection of the workplace at 1610 Huron Circle in Grafton, Wisconsin, to ensure employees re-roofing an apartment building utilized fall protection. This exposed employees to fall hazards of approximately 26 feet at the peak on the top roof and approximately seven (7 ) feet five (5) inches at the lower roof.

29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a)  On or about June 13, 2024, employees conducted re-roofing work on an apartment building, at 1610 Huron Circle in Grafton, Wisconsin, utilized a ladder for access/egress to a roof area that did not extend at least 3 feet above the landing, exposing an employee to a fall hazard of approximately 7 feet 5 inches.
