29 CFR 1910.28(b)(3)(iv): The employer did not ensure that each employee was protected from falling into a ladderway floor hole or ladderway platform hole by a guardrail system and toeboards erected on all exposed sides, except at the entrance to the hole, where a self-closing gate or an offset must be used.  a) Hal Welsh East Area Family YMCA, 200 Towne Drive, Fayetteville, NY 13066 on or about June 14, 2024: Employees were exposed to a fall height of approximately 62 inches when the top of the fixed ladderway used to access the pit was without protection from falling into the lower level. The ladderway did not have a self-closing gate and/or was not offset.  Abatement documentation is required.

29 CFR 1910.151(c):Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  a) Hal Welsh East Area Family YMCA, 200 Towne Drive, Fayetteville, NY 13066 on or about June 14, 2024: Employees were exposed to corrosive chemicals including, but not limited to, Clean All Swimpure Chlorinating Solution, which causes serious eye damage and skin corrosion, and Clean All Low Fuming Muriatic Acid, which causes serious eye damage and skin corrosion and were not provided with an unobstructed and/or maintained eyewash for immediate emergency use in their work area.   Abatement certification is required.

The electrical panel was blocked by a floor vacuum, garbage can, and box.
