Condition observed on or about 06/15/2024. b) 257 Mahwah Road, Mahwah NJ: (2) Employees engaged in roofing activities were exposed to fall hazards of approximately 15-18 feet to the ground below as a personal fall arrest harness was worn but not secured to an anchor. Condition observed on or about 06/15/2024.
