29 CFR 1910.244(a)(2)(i): Where there was a possibility of slippage of the cap, a block was not placed in between the cap and the load: On or about April 29, 2024, a 22-ton OTC jack was used to raise the front axle of a trailer to facilitate service. A block, or similar device including but not limited to an axle adapter, was not placed between the cap and the axle to prevent slippage of the cap.

The hospitalization was reported to OSHA on June 10, 2024.
