29 CFR  1926.501(b)(14):Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.  On or about June 17, 2024, during a construction project at a commercial building located at 6200 OAK TREE BLVD, INDEPENDENCE, OH 44131, the employer did not ensure an employee framing a window in preparation for a debris chute was protected from a fall of greater than 25 feet by the use of a guardrail system, a safety net system, or a personal fall arrest system.

On or about June 17, 2024, during a construction project at a commercial building located at 6200 OAK TREE BLVD, INDEPENDENCE, OH 44131, the employer did not ensure employee(s) framing a window in preparation for a debris chute demonstrated an adequate knowledge of the fall protection requirements to identify and implement conventional or personal fall protection while exposed to fall hazards in the work area.
