29 CFR 1926.652(b)(1)(i): Excavations were not sloped at an angle not steeper than one and one-half horizontal to one vertical (34 degrees measured from the horizontal) and no other options listed in paragraph (b) of this section were used. a) On or about June 27, 2024, the employer exposed employees to engulfment and crushing hazards, in that the trench was approximately 17 feet deep, and the top of the slope was approximately 19 feet out from the bottom of the trench, resulting in an angle steeper than one and one-half horizontal to one vertical.

29 CFR 1926.652(c)(2)(i): Design of support systems, shield systems, or other protective systems that are drawn from manufacturer's tabulated data were not in accordance with all specifications, recommendations, and limitations issued or made by the manufacturer. a) On or about June 17, 2024, the employer exposed employees to engulfment and crushing hazards, in that the steel road plates used as part of the protective system on the end of the trench boxes were not installed in accordance with the manufacturer's specifications.
